Instructions: The following is a list of symptoms people sometimes have. After each list of symptoms, place which number best describes how much that symptom or problem has bothered you during the past week. You should have a total of 33 answers. Once complete add up your score.

Zero = not at all, 1 = somewhat, 2 = moderately, and 3 = a lot.
